Pursuant to Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) Titanium Sp. z o.o. sp. k. with its registered office in Gdańsk informs:

WHO IS THE CONTROLLER OF YOUR PERSONAL DATA?

WHY DO WE PROCESS YOUR PERSONAL DATA?

Your personal data will be processed for health purposes, including prevention, medical diagnosis and treatment, the provision of social security, and the management of social security systems and services, as well as for certification, defense against claims, or marketing purposes.

LEGAL BASIS FOR THE PROCESSING OF PERSONAL DATA

Your personal data will be processed based on:

Article 6(1)(a) and Article 9(2)(a) of the General Regulation on Data Protection to the extent that your personal data are processed based on your consent to their processing, including, in particular, if the Company wants to encourage you to use its services or inform you about the current offer of services provided, as well as if it wants to boast about its achievements in the scope of services provided, including the publication of your image or a photo of your jaw before and after the execution of the works commissioned to the Company;

Article 6(1)(f) of the General Data Protection Regulation, about personal data obtained by recording your image on surveillance cameras in order to protect persons and property in the clinic;

Article 9(2)(h) of the General Data Protection Regulation, which shall apply concerning the following provisions of national law:

Article 3(2) of the Act on Medical Activity and Article 24 of the Act on Patient Rights and the Patient Ombudsman, when your data are processed for health prevention

Article 3(1) of the Act on Medical Activity and Article 24 of the Act on Patient Rights and the Patient Ombudsman, when your data are processed for medical diagnosis, treatment;

Article 54 of the Law on cash benefits from social insurance in the event of sickness and maternity or other relevant provisions of social insurance law, where processing is carried out to provide social security and manage social security systems and services

The purposes and legal basis for the processing of your personal data as described above mean that the Company will be able to process your personal data without your consent to keep medical records, to develop a diagnosis and a method of treatment appropriate for you, when it wants to inform you about pro-health events, when performing treatment services on your behalf, when a medical certificate needs to be issued, as well as in situations necessary to save your health or the life or health and life of others. You will only receive advertising material if you agree to this. Also, only with your prior consent will the Company use your image or a fragment of your face captured in photographs taken before and after treatment at the Titanium clinic for publication on its website or Facebook profile.

HOW LONG WILL YOUR PERSONAL DATA BE PROCESSED?

Your personal data will be processed (stored) for 21 years, and X-rays will be stored outside medical records for 11 years, in accordance with Article 29 of the Act of 06 November 2008 on Patient Rights and Patient Ombudsman of (Dz.U.2017.1318). In the case of data processing with your consent, personal data will be stored until your consent is withdrawn.

DO YOU HAVE TO PROVIDE YOUR PERSONAL DATA?

The provision of personal data by you is voluntary. However, suppose you do not provide personal data necessary for the provision of treatment services, prepare a diagnosis, and commence treatment. In that case, it will not be possible to conclude and carry out an agreement or the activities requested by you.

WHO WILL BE THE RECIPIENT OF YOUR PERSONAL DATA?

The recipients of your personal data will be authorized employees of the Company or other persons with whom the Company cooperates, including medical professionals, persons performing auxiliary activities in providing health care services, persons taking care of the safety and maintenance of the ICT system in which your data are processed, as well as persons who, on behalf of the Company, deal with the issue of settlements between you and the Company, who are obliged to maintain professional secrecy.

CAN YOU ACCESS YOUR PERSONAL DATA?

You have the right of access to your data, and the right to rectification, erasure, restriction of processing, right of transfer, right to object to processing. However, in view of the purpose of processing your health data, as well as the obligation to keep medical records, the right to restrict or transfer your data or to object to its processing is excluded concerning your data.

CAN THE CONSENT GRANTED BE WITHDRAWN?

You have the right to withdraw your consent at any time to the processing of your personal data that the Company is processing because you have previously given your consent. The operations on your personal data carried out until the moment of revoking your consent to their processing remain following the applicable law. You have the right to lodge a complaint with the President of the Office for the Protection of Personal Data if you believe that the processing of your personal data concerning you has infringed the provisions of the General Data Protection Regulation or national law

WILL YOUR DATA BE TRANSFERRED TO A THIRD COUNTRY?

Your data will not be transferred to a third country or an international organization.

WILL AUTOMATED DECISIONS BE MADE BASED ON YOUR PERSONAL DATA?

The Company does not make automated decisions based on your personal data, including on profiling.

Cookies Policy

The titaniumdental.pl website uses “cookies, ” allowing for remembering selected information about its users and their preferences. However, they are not used to process or store personal data, but only information that allows:

  • disabling the banner informing about cookies on the website;
  • creating statistics on the portal’s audience and the effectiveness of advertising campaigns on the Internet;

Titanium Sp. z o.o. Sp. k., does not use cookies to contact you by mail, e-mail, or phone.

What are cookies?

The titaniumdental.pl website uses two types of cookies:
